You understand that your consultation will be conducted via phone, video, or other telecommunication technologies and may not involve a physical examination. As a result, the accuracy of diagnosis and/or treatment may be limited. This consultation is not a substitute for an in-person medical evaluation.
You agree to seek in-person care, emergency services, or specialist follow-up as directed by the physician or if your symptoms worsen. You understand that virtual care is not appropriate for medical emergencies.
While we use secure platforms, you acknowledge that there are inherent risks associated with telecommunication technology, including technical failures, interruptions, or potential unauthorized access. By proceeding, you accept these risks and release our practice from liability related to technology-based issues beyond our control.
Southern California Brain & Spine Surgery complies with HIPAA and takes reasonable precautions to protect your medical information. However, by using telehealth platforms, you understand that absolute confidentiality cannot be guaranteed.
By proceeding with a virtual consultation, you confirm that you are participating voluntarily, that you understand the nature and risks of remote care, and that all questions you may have had about this process have been answered.
To the fullest extent permitted by law, you release and hold harmless Southern California Brain & Spine Surgery, its physicians, staff, contractors, and affiliates from any and all claims, liabilities, or damages arising out of or related to your participation in a virtual consultation, except in cases of gross negligence or willful misconduct.
This Agreement is governed by the laws of the State of California. Any legal disputes shall be handled in the appropriate courts of Los Angeles County, California.
